I, the undersigned, hereby agree to the following:
1. I am voluntarily participating in Zumba classes. I hereby affirm
that I am in good physical condition and do not suffer from any known
condition which would prevent my participation.
2. I knowingly, voluntarily, and expressly waive any claim I may have
against the Zumba instructor for injury or damages sustained as a result
of participation in Zumba.
I have read this waiver of liability & informed consent and fully understand
its terms. I understand that I am giving up my right to sue. I acknowledge
that I am signing the agreement freely and voluntarily, and intend my
signature to be a complete and unconditional release of all liability to the
greatest extent allowed by law in the State of West Virginia
